Instructions for electronic documents submitted to FRSQ

To access the ftp server, you will need a username and a password. To obtain your personal login information, please send an e-mail to genomics@frsq.gouv.qc.ca with the following information:

-Full name of Co-Principal investigator from Quebec

-FRSQ PIN of the Co-Principal investigator from Quebec

-E-mail of the Co-Principal investigator from Quebec

-Full name of Co-Principal investigator from China

Your username and password will be sent to you by e-mail along with detailed instructions on how to access the ftp server.

Please respect the following requirements:

-Fonts accepted:

-Arial, size 11;

-Times New Roman, size 12;

-Calibri, size 12.

-Single line spacing

Format and name

The completed proposal must be saved in Word format.

The document must be named respecting this nomenclature:

Family name of Quebec Co-Principal Investigator_Family name of China Co-Principal Investigator_Genomics_Application

Ex: Tremblay_Chen_Genomics_Application.doc

Submission

The Quebec Co-Principal Investigator must submit the proposal to the FRSQ on the server ftp://genomics.frsq.gouv.qc.ca

Research on Genomics and Diseases

For all applicants 

-CVs from all the investigators (in the format prescribed by the research funding agencies).

-For Quebec researchers: Canadian Common CV, full version, fully completed, and including detailed contributions

-For Chinese researchers: Biographical Sketches

For Quebec applicants only

Scan the documents in pdf format, if necessary, and upload them on the server.

-The completed Suggested Reviewers Form

-Clinical investigators from Québec must prove that they are members in good standing of the professional corporation that governs them in Québec, that they have the right to practice in Québec, and that they have professional liability insurance.

-A copy of the permanent resident card, where applicable, to be received before the starting date of the grant or at the latest by June 30, 2011

-A letter of endorsement from the director of the institution or of the university department where the research will be conducted

(10 a.m., Montréal time or 10 p.m., Beijing time)

Please ensure that the full proposal and all required documentation are uploaded to the appropriate electronic systems by the prescribed deadline so that it can be processed.

FRSQ accepts no liability for incomplete applications.
